78701- Kidney imaging morphology; with vascular flow
For kidney imaging with vascular flow test, the radionuclide used is 99mTc- DTPA or MAG3. This is the most commonly used radioactive agent. For DTPA scan, the patient lying on the scanning bed with gamma camera under the bed. A small injection is given in the arm and a cannula is inserted into the vein. Then a radiopharmaceutical agent is injected. The camera will detect the agent and will produce images of the kidneys. This radioactive agent follows the blood as it flows through the kidney and finds any obstruction in the renal collecting system to check the filtration rate of kidney.

78707- Kidney imaging morphology; with vascular flow and function, single study without pharmacological intervention

This exam is same as Kidney imaging with vascular flow test. The only addition is to check the functions of kidney. Therefore, this exam checks both the vascular flow .i.e. blood flow and the function of kidney. The proper functioning of tubules and ducts and the rate of kidneys are filtering blood, are the common things to check through this exam.

Procedure for CPT Code for Renal Scan with Lasix

78708- Kidney imaging morphology; with vascular flow and function, single study, with pharmacological intervention (e.g., angiotensin converting enzyme inhibitor and/or diuretic)
The exam is also called Lasik (scan). This exam is same as Kidney imaging with vascular flow and function. The only addition is the use of pharmacological intervention. The imaging of kidney is same as done in procedure 78707. If pharmacological intervention is used, a diuretic may also be injected to increase the flow of urine during the exam. The commonly used diuretic is Furosemide (Lasix). This makes kidney to make more urine and decrease the amount of water from reabsorption. Increase flow of urine in the ureter helps in detecting any obstruction in the ureters. Surgery may be necessary to clear the radiotracer if kidney fails to clear it after diuretic administration.

PROCEDURE: Lasix renal scan with diuretic washout,
10.0 mCi technetium 99m MAG3 was injected. Imaging of the kidneys was performed for 20 minutes. The patient then received 20 Lasix intravenously, and imaging was continued for a total of 60 minutes.

FINDINGS:
Perfusion to the kidneys is prompt and symmetric. There is prompt right renal uptake and excretion with complete washout following Lasix administration. Left kidney shows prompt tubular function and transit to the renal pelvis. Following the administration of Lasix there is increasing activity in the renal pelvis with minimal washout. This represents obstruction and is at the ureteropelvic junction. The split function is 45.6% on the left and 54.4% on the right.
