Thoracentesis, is a minimal invasive procedure to remove fluid or air from the Pleural space diagnostic or therapeutic purposes. In medical coding, thoracentesis cpt code is frequently used. Hence, learning thoracentesis cpt code is necessary. After administration of local anesthesia, a hollow needle is inserted in the thorax. Patient sits upright and leans on a table. The physician removes fluid and/or air from the chest cavity by puncturing through the space between the ribs with a hollow needle (cannula) and entering the chest cavity. Through the cannula or hollow needle the extra fluid from the pleural space drained into a bag. These tips are really important for Certification exams like CPC, you can go through sample CPC questions here which will surely help in clearing CPC exam.
Accumulation of fluid in pleural space is called as pleural effusion. The most common causes of pleural effusion are pneumonia, cancer and congestive heart failure.

Thoracentesis and pleural drainage have new CPT codes. Now we will not report 32554-32557 along with 76942, 77002, 77012, 77021 and 75989. There are new codes for thoracentesis and pleural drainage which include imaging guidance. So, will code only one CPT code for Thoracentesis with imaging guidance. Hence, we have new bundled codes for thoracentesis with or without imaging guidance.
Procedure description
Procedure code without imaging guidance
Procedure code with imaging guidance
Thoracentesis, needle or catheter, aspiration of the pleural space
Pleural drainage, percutaneous, with insertion of indwelling catheter

To report insertion of indwelling tunneled pleural catheter with cuff please code cpt 32550.

Procedure included with Thoracentesis cpt code

Moderate sedation CPT Codes are not included with these CPT codes 32554-32557. Moderate sedation should be coded separately when these services are provided.

For Pleural drainage percutaneously, the physician removes fluid and/or air from the chest cavity. The patient is positioned dependent upon the location of the fluid (blood or pus) on preprocedural imaging. The chest cavity is accessed by puncturing through the space between the ribs with a needle. Once the fluid is accessed via the needle(s), a guidewire is inserted to the most distal point of the fluid in the pleural cavity. A catheter (i.e., pigtail catheter) is inserted the length of the guidewire and the chest cavity is drained. In some instances, the catheter is connected to a water seal system.

When thoracentesis is done on both sides of the chest, we can code 50 modifier with CPT code 32554-32555. For open tube thoracotomy, please report code 32551.

Same happens with Pleural drainage CPT codes, if done on both sides we can use 50 modifier for bilateral procedure with CPT code 32556-32557.

Never, code an imaging guidance code with the CPT code 32555 and 32557. These CPT codes include imaging guidance hence no need to code it separately.
