Lumbar puncture is done to collect the cerebrospinal fluid from the lumbar portion of the spinal canal. The spinal fluid obtained through lumbar puncture helps in diagnosing brain hemorrhage, pressure from hydrocephalus, encephalitis, meningitis and brain or spinal cord tumors. Lumbar Puncture, Diagnostic

Indication/diagnosis: Intracranial tumor.

Technique: The procedure, risks, indications and alternatives were explained to the patient's daughter who provided verbal consent. The patient was deemed unable to provide his own consent..

Contrast: 0 mL contrast

The back was prepped and draped in the usual sterile fashion. Local anesthesia was obtained with 1% lidocaine. Under direct fluoroscopic visualization a 22-gauge needle was advanced via a right-sided sublaminar approach at L5. The needle was easily advanced into the thecal sac using this approach. Opening pressure was not obtained.  The needle was removed. Patient tolerated the entire procedure well with no immediate complication.

Fluoroscopy time: 1 minute of fluoroscopy time was used.

Specimen: 10 mL clear CSF sent to the laboratory for analysis.

Estimated Blood Loss: Less than 1 mL

Impression:
1.Technically successful fluoroscopically-guided lumbar puncture.

Hi Jitendra I have small doubt please clarify, If lumbar puncture is done CSF fluid is collected and opening pressure was measured and then which CPT we have to code?
If lumbar puncture is done and chemotherapeutic(methotrexate) agent is administered for neoplasm treatment then what code we have to code?

As per my knowledge, the first scenario will be coded with diagnostic lumbar puncture cpt code and second one coded with therapeutic lumbar puncture CPT code...hope this clarifies your doubt..

that is a good question for lumbar puncture CPT code..in simple language if a small amount of CSF fluid is removed for studying we used to code diagnostic procedure 62270 but if the CSF fluid is removed in large amount or drained from the body we code it as therapeutic procedure for lumbar puncture in interventional radiology 62272...for CPT code 96450 the lumbar puncture is included since the procedure is done to treat the diagnosis by injecting methodextrate for chemotheraphy for neoplasm in this lumbar puncture is done with or without removing CSF fluid but the sole objective is to inject the chemothepeutic agent..hope this clarifies your doubt

When performing a diagnostic lumbar puncture and therapeutic through the same puncture, you would only report the therapeutic lumbar puncture CPT code for higher rvu's.
