Causes of Back pain?

Back pain is generally occurs in the spinal region. Spinal region includes cervical, thoracic, lumbar and sacral region. The spine is a interconnecting network on nerves, muscles, joints, tendons and ligaments and all of which are capable of producing pain. Back pain may be acute or chronic. There are many sources of back pain such as spondylosis, spinal disc degeneration, degenerative disc disease, spondylolisthesis, osteoarthritis, spinal stenosis, trauma, inflammatory diseases etc.Hence, icd 9 code of back pain is more frequently used as diagnosis code.

Coding of back pain in medical coding field is little tricky, since there there are different codes for the different part of spinal region. Cervical spine from where the vertebral column starts and then the thoracic then lumbar and then finally the sacral region. These all parts of vertebral column have different ICD 9 code for pain.
For general icd 9 code for back pain, one always code for 724.5 and for ICD 10 the code is M54.9 which states backache unspecified. Since the back pain doesn't specify whether it occurs in lumbar or thoracic region we tend to code unspecified code for back pain to be on safer side. Also most of the times we have to code only according to the documentation, hence mostly we go with 724.5.
Now while coding some charts some physician mention upper back pain or middle back pain or lower back pain. So while coding the charts with such symptoms in the beginning of the chart leads to same unspecified code, but while coding most of take the site specific pain of the vertebral column.
For upper or middle back pain we mostly code 724.1, thoracic pain, since the middle vertebral column is generally consist of thoracic spines. For lower back pain or lumbago it always leads to 724.2 for the lumbar spine pain, since the lower part of vertebral column is mainly of lumbar spines.
Hence we can now be able to code back pain easily as per documentation

only back pain- 724.5 (M54.9)
upper or middle back pain- 724.1 (M54.6)
lower back pain or lumbago- 724.2(M54.5)

Now, there are some codes which include these back pain codes. Yes, I am talking about radiculitis. Radiculitis is radiating pain going from cervical to upper arm or from thoracic or lumbar to lower extremity. Hence, radiculitis being a radiating pain will always include the pain of that region.
Hence, the icd 9 code for this radiating pain also differs with the anatomic region. So when there is cervical radiculitis we code 723.4 for radiation cervical pain to upper limb. For thoracic or lumbar radiculitis we code 724.4 for radiating pain for thoracic or lumbar to lower limb. Also remember don't code any spondylosis code of spine with the 724 series codes.

723.4- cervical radiculitis
724.4- thoracic or lumbar radiculitis

M54.12 - Radiculopathy, cervical region
M54.13 - Radiculopathy, cervicothoracic region
M54.14 - Radiculopathy, thoracic region
M54.15 - Radiculopathy, thoracolumbar region
M54.16 - Radiculopathy, lumbar region
M54.17 - Radiculopathy, lumbosacral region



Also, spinal stenosis which is one the major reason for back pain also has different icd 9 code. Lumbar spinal stenosis is coded as 724.02 and cervical spinal stenosis is coded as 723.0. Back pain is also caused by spinal disc degeneration and osteoarthritis. Osteoarthritis of spine leads to spondylosis hence the code will be same as per anatomic site,

spondylosis of lumbar spine- 721.3
spondylosis of thoracic spine- 721.2
spondylosis of cervical spine- 721.0
